The findings said the incident was "foreseeable and preventable" and that the Secret Service and other security agencies had multiple failures in planning, communications, security and quotas. Gary Peters, a Michigan Democrat and chairperson of the Senate Homeland Security and Governmental Affairs Committee, said the consequences of these failures were dire. Investigators found that there was no clear chain of command between the Secret Service and other security agencies. In addition, officials used multiple independent radio channels to operate, resulting in communication outages.
